Dear Burbank Families,
The Burbank Coordinating Council would like to offer you the opportunity to participate in our 2021 Holiday Basket Program, which provides food and gifts for your family in December.
If you qualify for free or reduced priced lunches, and you would like a basket for your family, please fill in the Google Form on our website BEFORE November 21, 2021. We will process applications on a first-come, first-served basis, so don’t wait to apply.
Baskets will be distributed on Saturday, December 11, 2021, between 9:00 am and 12:00 pm.
Once we have reviewed and accepted your application, we will call you with the pickup location for your basket.
To be considered:
You must apply online before the deadline. One application per family only.
Your email and phone number must be valid, and you must call us if it changes.
An adult from your family must pick up and sign for your basket.
If you don’t follow these steps, you may not get a basket.
Instructions for completing your application online- this is the preferred method:
Fill in the information in the Google Form. You must fill in all fields.
If you have special needs or concerns, there will be a place on the Google Form for you to share with us.
Click submit and you are done.
